The Tax Organizer

Include the following:

1. Print and fill out the Tax Organizer completely. You'll need Adobe Acrobat Reader to view and print the Organizer. 
2. Send all W-2's for you and your spouse.
3. Send all 1099 and 1098 forms (from banks for interest, dividends, and mortgage interest paid). Also, any 1099B forms (from stock sales outside of your 401(k)). For stock sales, please
   include your cost basis, date acquired and date sold.
4. A copy of your prior year's tax return, both Federal and State (if applicable) - if we didn't prepare them for you.
5. For electronic filing, after your return has been completed, you must download, print, and sign the completed Form 8879 we will send you. Then fax or mail this completed form to us to authorize your e-filing. We will send this form to you filled out with your return information
   for you to sign after the return is complete.

Mail everything via UPS, FedEx, or United States Postal Service Certified Mail to Flying Filers at the address found in the Tax Organizer packet and found below. If you are in Chicago, you can drop your information off in person anytime during the business day at 5715 W. Grace St through our secure mail slot.  

• We do not recommend requiring Signature Upon Delivery, doing so WILL DELAY THE PROCESSING OF YOUR RETURN.  We have secure mail slot for secure deliveries of mailed documents.

We request paying for the Basic Return fee in advance to speed your return!  If additional Forms or Schedules are required, we will invoice those additional costs prior to finalizing your return. 

Payment for services is quick and easy using the PayPal link in the Payment section.  You can mail a check for payment with your return as well payable to Flying Filers.
